Work Order Request
A computer and/or telephone related work request can be submitted by completing the LSCO Help Desk form.
A Department of Information Services work request form should be completed for computer related problems, moving telephones, changing telephone numbers, administrative computing access (BANNER), network access, moving computers, and/or forgotten password(s).
Based on the information given, the work request will be submitted for processing. Work requests are processed based on a combination of priority, date requested and staffing resources.
